Khi sử dụng POCUS để đánh giá chức năng thất phải (RV) hoặc tim phải, rất có thể bạn đang xử trí một bệnh nhân nguy kịch — chẳng hạn như bệnh nhân thuyên tắc phổi, tăng áp phổi, suy tim phải, hoặc thậm chí nhồi máu cơ tim thất phải. Trong những tình huống có tính quyết định cao như vậy, bạn không có thời gian để chờ một kết quả siêu âm tim toàn diện từ bác sĩ tim mạch. Bạn cần câu trả lời nhanh chóng. Đó là lý do vì sao người thực hành lâm sàng cần hiểu rõ sinh lý thất phải, đánh giá được bệnh lý thất phải và thực hiện một đánh giá thất phải đáng tin cậy ngay tại giường.

Tuy nhiên, đánh giá tim phải và thất phải bằng POCUS và siêu âm tim không hề đơn giản. Không giống thất trái (LV), vốn có hình dạng tương đối đơn giản và gần tròn, thất phải là một cấu trúc phức tạp, có hình lưỡi liềm và cần được đánh giá trên nhiều mặt cắt khác nhau. Vị trí sau xương ức khiến việc thu nhận hình ảnh trở nên khó khăn; và mặc dù có thể đo nhiều thông số, không có một giá trị tiêu chuẩn vàng duy nhất để xác định chức năng thất phải. Hơn nữa, hiện chưa có một quy trình đánh giá thất phải nào được chấp nhận rộng rãi, điều này càng làm tăng tính phức tạp. Tuy nhiên, trong hướng dẫn này, chúng tôi sẽ trình bày một phương pháp cơ bản để đánh giá thất phải theo từng bước, sử dụng cả các phương pháp định tính và định lượng.

1. Giải phẫu thất phải và các mặt cắt siêu âm

Các mặt cắt thường được sử dụng nhất để đánh giá chức năng thất phải gồm mặt cắt trục dài cạnh ức, mặt cắt cạnh ức trục ngắn cạnh ức (ngang mức cơ nhú) và mặt cắt 4 buồng qua mỏm. Mặt cắt dưới mũi ức cũng có thể được thực hiện khi không thu được các mặt cắt cạnh ức hoặc từ mỏm. Để xem hướng dẫn đầy đủ về tất cả các mặt cắt siêu âm tim, hãy xem tại đây.

1.1. Mặt cắt trục dài cạnh ức (PSLA)

Tim nằm theo hướng chếch, với thất phải nằm phía trước thất trái. Mặt cắt trục dài cạnh ức (PSLA) được sử dụng để đánh giá kích thước thất phải so với kích thước thất trái.

1.2. Mặt cắt trục ngắn cạnh ức (PSSA) qua cơ nhú

Mặt cắt trục ngắn cạnh ức (PSSA) cho hình ảnh cắt ngang của tim. Mặt cắt PSSA nên được thu nhận ở mức cơ nhú để đánh giá tỷ lệ kích thước thất phải/thất trái (RV/LV), đồng thời đánh giá dấu hiệu chữ D (D-sign).
